Care providers available: Registered nurse, professional counselor, clinical case management, outreach worker through Supported Independent Living Program.

Services available: Medication assistance through RN, counseling services, completion of disability and assistance forms through counselor, AA group once per week.

Application process: Direct and self-referral.

Mon to Fri: 8:30 am to 4:30 pm
