Care providers available: Clinician case managers, community support worker, outreach psychiatrist visits every 6 to 8 weeks for assistance with psychiatric medications. Services provided: Assistance with disability/assistance forms, but need input from family doctor, counselling services, prevention worker offers day programs on fluctuating basis, such as Changeways. Application process: Direct and self-referral.
