Care providers available: Possible access to GP through urgent care clinic, psychiatrist once per week, registered nurse, nurse practitioner, registered social worker, professional counsellor, mental health outreach worker, mental health community worker.

Services: Clinical case management, community-based case management, outreach, medication assistance, monitoring of lab results, counselling services, completion of disability and assistance forms for known clients, housing stabilization.

Application process: Direct and self-referral. Areas served: Parksville, Qualicum, Bowser, Errington, Coombs, Nanoose Bay, Lasqueti Island.
