Mental health services. Case management services for adults with severe and persistent mental illness.

Care providers available: Psychiatrist (GP referral required), registered nurse, nurse practitioner, registered social worker, professional counselor, outreach worker, community worker.

Services: Clinical case management, community-based case management, outreach, medication clinic, psychiatrist will review lab results for clients, counselling services, 4-week groups on anger management, anxiety, several times per year.

Application process: Direct referral. Apply through Intake staff at this location. Referral can be made by family doctor.
