Care providers available: Psychiatrist once per month, registered nurse, registered social worker, mental health outreach worker, mental health community worker.

Services provided: Clinical case management, community-based case management, outreach, medication assistance, counseling services, completion of disability forms.

Application process: Self-referral. Doctor’s referral required to see psychiatrist.
