Care providers available: Psychiatrist, psychologist available on intermittent basis, registered nurse, registered social worker, mental health outreach worker, addictions worker, occupational therapist.

Services: Clinical case management, outreach, medication assistance through case manager or ACT Program, weekly support groups for mental health/addictions issues, housing stabilization if connected with a counsellor.

Application process: Direct and self-referral.
Area served: Port Alberni, Uclulet, Bamfield, Tofino and communities in between.
